File contains original documents related to the seminar which were provided by Professor Owen Wade. Amongst the documents are Professor Owen Wade's own records of the first meeting of the Committee on Safety of Drugs ("CSD") and these records are referred to in footnote 97 on page 46 of the published volume. Documents include;

Covering letter from Owen Wade dated 9 January 1997 provided with some of the documents being deposited.

Copy of the judgement of Justice Webster (High Court of Justice, Queen's Bench Division), in the case of The Queen vs The Committee on the Review of Medicines: Ex parte E.R. Squibb & Sons Limited. The judgement is dated 8 April 1981.

A copy of the affidavit provided by Professor Wade in the above court case and a covering letter from the solicitor's office at the Department of Health and Social Security, dated 14 January 1981.

First "yellow" warning pamphlet, no.1, February 1964, produced by the CSD in its Adverse Reaction Series and titled "Monoamine Oxidase Inhibitors".

Second "yellow" warning pamphlet, no.2, August 1965, produced by the CSD in its Adverse Reaction Series and titled "Early Warning System".

Third "yellow" warning pamphlet, no.3, August 1965, produced by the CSD in its Adverse Reaction Series and titled "Some Anti-arthritic Drugs and a New Analgesic".

Fourth "yellow" warning pamphlet, no.4, January 1967, produced by the CSD in its Adverse Reaction Series and titled "Chloramphenicol".

First CSD meeting minutes, held 6 June 1963.

Owen Wade's chapter 13 (book title not mentioned) titled, "Adverse Reactions to Drugs, The Government, The Media and the Law".

A paper titled, "Twenty Years on from the Dunlop Committee".
