Register of Patients (Formal and Informal)

The register contains admission number, patient name, dates of admission and discharge and observations. Further columns which are inconsistently filled relate to the due date for Forms B and Form 7 and which Section (3/4/5) patient is classed in. The first seventeen patient names listed cover admission dates 1944-1981 for patients resident on 1 January 1982, with the remainder of the volume covering admissions for 1982 and 1983. On the inside cover of the volume a typed notice is attached giving instructions as to the completion of the register. The register was discontinued in December 1983.

Sections 3, 4, and 5 refer to Sections of the National Health Service, with Section 3 being patients receiving free treatment and Sections 4 and 5 being fee paying patients at various rates.
