Description

Completed duplicate forms issued under the Public Health (Infectious Diseases) Regulations (Scotland), 1932. Forms record patient name, name of hospital at which they are resident, name of disease they are suffering from, age and sex of patient, date of onset of disease, signature of Medical Practitioner, address and date. There are sections relating to various diseases including Tuberculosis, Ophthalmia Neonatorum, Malaria, Dysentery, Puerperal Fever and Puerperal Pyrexia.
