Applications for registration 1962-1965

Set of early registration forms, detailing the names of parents, address, name of child, how affected, name and address of family doctor and of hospital where treated. Mostly in alphabetical order.

Also found with the material were lists of members' children, one of them organised by DOB and/or months of birth, possibly with a view to sending birthday cards. Another (alphabetical) list records name, DOB and how the child is affected. This, along with the way registration forms were structured, indicates a marked interest of the Society in medical information.
