Evaluation sheets (daily care record) recording Ismond Rosen's condition, medication, care and treatments during the final months of his illness. The care sheets cover 3 April until his death on 16th October 1996. There is also some other material relating to his care including a chart to identify specific needs or problems and various mobility and care aids.
