This item, and SA/MIN/E/2/2-4, comprise MindLink membership monitoring forms completed in 1997; the forms, which were numbered by MindLink and are in numerical order (with gaps), have been split into four catalogue items in order to produce orderable items of more manageable size. The forms numbered 4,5,8,9,12-15, 17 and 18 included in this item were at the top of the original stack of forms, separated from the main body of forms by a small amount of general correspondence, which has been catalogued as SAMIN/E/3/1/14. A small number of forms dating to 1998 found at the bottom of the stack have been catalogued separately as SAMIN/E/2/1/5, due to Data Protection measures requiring their closure for an additional year.
