Acc 612 was given to the Contemporary Medical Archives Centre by Dr C I Pelton, of the Health Centre, Victoria Avenue, Wellington, via Dr Michael Bevan of the Wellcome Unit for the History of Medicine, Oxford, in January 1996 and Acc 1235 directly by Dr Pelton to Archives and Manuscripts in April 2004
