Register of Death and Burial

This register contains name of patient, reference number, ward, home address, date and time of death, signature of mortuary attendant, date and time of removal, name and address of person arranging the funeral, name and address of undertaker, place of burial, signature and address of person checking and removing body and date of removal. Attached to the inside cover are notes relating to the burial arrangements for two patients.
