Contains date of admission, number in civil register (see H64/B/01), date of discharge, transfer or death, patient's name, whether private or pauper, marital status, occupation, number of times certified as 'Insane', ages on present admission, on onset of present attack, and on first attack, whether the attack was a first attack or not and if congenital, number of previous recoveries elsewhere, duration of present attack, principal and contributory factors (Aetiological), bodily state on admission, any bodily diseases, instance of epilepsy, 'form of mental disorder', if deemed suicidal by Medical Officer and observations (for example 'urgency' case).

Biographical note

In accordance with the Rules of the Commissioners in Lunacy, 31 October 1906 number 822, under rule 5: 'The entries in the medical register, other than those which are duplicate entries of those in the civil register made on admission by the clerk, shall be supplied by the medical officer of every asylum, hospital or house within three months from the date of reception of the patient'.
