Description

Each card gives a brief summary for each patient recording the following information: name, age, admission dates, case number, case book number and page (for the case books see H64/B/06). Also whether a new case, urgency or transfer, diagnosis, duration of attack, number of previous attacks, whether in the hospital before, cause, times discharged, recovered or died, whether suicidal, duration of attack previous to certification, marital status, religion, profession.

On the reverse the cards give: date recovered, discharged where to or died, 'form of mental disorder' on admission, age, total length of the attack and whether treated at Saint Luke's Hospital's, Welders or Nether Court sites.

H64/B/19/026 contains the instruction cards which were used by staff when the cards were completed and give the guide to the letter code which were used to fill in the cause.

Although the card index began in circa 1900, the index was back-dated include patients present at the hospital who has been admitted previous to 1900 and therefore covers admissions before 1900.
