Author's signature dated 1917 inside cover. He qualified M.D. at University College, London, in 1900 and was elected F.R.C.P. in 1909. He was physician to the Shadwell Hospital, and to the City of London Hospital for Diseases of the Chest [cf. B.M.J. 1929, i. pp. 526, 527].
