These generally include name, sex, age, familial relationships (e.g. husband of, number of wives if male, wife of or child of), village, notes on condition (up to a paragraph), how long patient has had condition, type of leprosy, some free hand diagrams. Mixture of adults and children. Case number written in red ink.
