Description
The information entered into these books changes over time:;H64/B/01/001 - 010 (1751 - 1828): contain brief entries of the patient's name, parish of abode, legal settlement, name of person recommending the patient, date petition was delivered, date of admission, whether parish poor, date of discharge and brief reason for discharge - for example 'cured', 'uncured', 'died', 'unfit'.;On 1 August 1828 Act of Parliament George IV Chapter 41 was passed which required a change to the way patients were recorded:;H64/B/01/011 - 014 (1828 - 1845): contain date of admission, date of Certificate of Insanity and by whom signed (from 1832 this was replaced by a Medical Certificate), patient's name, age and marital status, residence and occupation, by whose authority the patient was sent and their residence (in many cases this was a relative), whether 'Parish Poor' '(P.P.)', whether 'found a lunatic by Inquisition and date', date discharged, whether cured, uncured or other reason, and if they died.;H64/B/01/015 - 020 (1845 - 1890) : give date of last previous admission, date of admission, patient's name, sex and class (whether private or pauper), age, marital status, occupation, previous place of abode, 'County, Union or Parish, to which Chargeable', by whose authority sent, dates of Medical Certificates, and by whom signed, bodily condition, 'name of bodily disorder (if any)', 'form of mental disorder', 'supposed cause of Insanity', whether 'epileptic' or 'Congenital Idiot', duration of existing attacks, number of previous attacks, age on first attack, date of discharge or death, form of discharge (whether recovered, relieved, not improved), if death observations on cause of death. They also give the payment rate.;In accordance with the Rules of the Commissioners in Lunacy 29 March 1890 article 1 paragraphs 1 and 2:
7 (1): 'The clerk of every asylum and the manager of every hospital...shall, immediately on the reception of a person as a lunatic, make an entry with respect to such lunatic in the Register of Patients, according to the prescribed form...' (2) 'the entry as to the form of disorder shall be supplied by the medical officer of every asylum within one month, and by the medical officer of every hospital...within seven days after the reception of the patient'.;H64/B/01/021 - 024 (1890 - 1906): give date of last previous admission, date of admission, date of Continuation of Reception Order, patient's name, sex, age, marital status, occupation, previous place of abode, 'County, Union or Parish, to which Chargeable', by whose authority sent, dates of Medical Certificates, and by whom signed, form of 'mental disorder', bodily condition and name of disease if any, 'supposed cause of Insanity', whether 'epileptic' or 'Congenital Idiot', duration of existing attacks, number of previous attacks, age on first attack, date of removal, discharge or death, form of discharge (whether recovered, relieved, not improved), if death observations on cause of death. They also give the payment rate.;In accordance with the Rules of the Commissioners in Lunacy 31 October 1906: rule 4 'The clerk of every asylum, hospital...shall, immediately on the reception of a person as a lunatic, make an entry with respect to such lunatic in the Civil Register of Patients, according to the form therein...';H64/B/01/025 - 027 (1907 - 1916): civil registers give date of previous admission (if any), date of admission, date of reception or urgency orders and continuation of order, whether a direct or transfer, whether any irregularity or lapse in orders, patient's name, whether private (criminals marked in red) or pauper, usual place of abode and from whence brought, sex, civil state (marital), religion, Union, County or Borough to which chargeable, by whose authority sent and name and address of petitioner (if any), dates of medical certificates and by whom signed, date of discharge, transfer or death, form of discharge (recovered, relieved or not improved) and related observations.;The dates given in this catalogue refer to the admission dates.
