Description

165 patient record cards containing notes of consultations, prescriptions and charges paid and due, 1904, 1916 and 1939. The cards give patients' names and addresses.

A card was filled out for each patient, recording dates of consultations, treatments prescribed and charges paid and due. A note on the envelope in which the cards were sent to the RCGP states that they are 'Record cards as used at the new House, Ross-on-Wye, 1904-1946 - A new card each quarter!! (every 3 months) for quarterly accounts' and also notes 'Bad debts negligible'. The reason for the survival of cards only from these years is unknown.

Notebooks of this practice, 1861-c.1940s are held as WMSS 7521-7523.

Acquisition note

These items were transferred to the Contemporary Medical Archives Centre in September 1997 from the Royal College of General Practitioners, which had received them in 1978 from Dr Marner Lloyd of Ross on Wye.
