Form letter submitting needy patients for consideration for treatment by the Surrey Dispensary within the Borough of Southwark and neighbouring boroughs in the 1780s, either at the dispensary or in their own 'habitations'. 8 rules are given and the following doctors who worked there are listed: Thomas Dale, William Hawes, William Hamilton, Henry Fearon, James Simpson.
