Charles Rodeck from King's College Hospital discusses fetoscopy and fetal blood sampling. The following summary accompanies the cassette: "Some of the uses of blood sampling and fetal examination by foetoscopy are mentioned. The Needlescope is demonstrated and the technique for its use is described. Real-time scanning immediately before the insertion of the fetoscope is an essential part of the procedure. It ensures selection of the optimal entry-point so that damage to fetus and placenta is avoided, and access is gained to the fetal region to be examined or to the cord insertion for blood sampling. A particle-size analyser provides immediate information on the quality of blood specimens. A film sequence shows a fetus being examined and blood being sampled. With this technique, pure fetal blood can be obtained in over 95% of cases when the sampling is performed between 18 and 22 weeks gestation. The results of diagnostic cases are shown and the risks discussed."

Mr Geoffrey Chamberlain in discussion with Dr Gordon Stirrat on the subject of pre-eclampsia in pregnancy. They emphasise throughout that pre-eclampsia is a little understood condition, frequently misdiagnosed. They pick through known research, debunk many pre-existing ideas relating to the diagnosis of pre-eclampsia based on hypertension alone, and attempt to piece together a clearer picture of the condition, its diagnosis and the best forms of treatment.

The management of pre-eclampsia.

Joseph Jordan of Birmingham Maternity Hospital, in discussion with Geoffrey Chamberlain, Institute of Obstetrics and Gynaecology, London. Firstly, Jordan attempts to show whether or not death from squamous carcinoma of the cervix is preventable. He describes how squamous carcinoma is usually, if not always, preceded by a pre-cancerous lesion known as carcinoma in situ, and explains how regular screening by cervical cytology (Pap test) may help to reduce mortality from this disease. Explanations are also suggested which attempt to show recent research into how and why carcinoma of the cervix arises in the first place.

Epidemiology of carcinoma in situ of the cervix.

The following summary accompanies the cassette: "Antisperm antibodies may occur in men or women as a result of exposure of the sperms to systemic immune cells. If the quantity of antisperm antibodies formed is sufficient then they can interfere with fertility. Men usually get much higher titres of antibodies than women, and it has been shown that high titres of antisperm antibodies are the primary cause of infertility in approximately 10% of sub-fertile men. The situation in women is far more complex, where low titres of antisperm antibodies are common and where false positive results make interpretation difficult. The different tests available to detect antisperm antibodies are described, and a method of checking the result of the test is defined. The essential difference between naturally infertile men and men following vasectomy is discussed and the probably nature of future research in immunological infertility is outlined."

Immunology of infertility.

A discussion between John Studd from King's College Hospital Medical School and Mr. Geoffrey Chamberlain. The following summary accompanies the cassette: "Following ovarian failure there is a 13-fold increase in plasma FSH and a 3-fold increase in luteinizing hormone together with a fall in oestrone and oestradiol to 20% of their normal follicular phase values. Oestrogen therapy will remove most of the symptoms of the climacteric but there is no doubt that unopposed oestrogens carry a risk of hyperplasmia and possibly endometrial carcinoma. Hyperplasia can be prevented by giving 10 days of a progestogen with the cyclical oestrogen therapy and this should be the recommended regimen of therapy. If cystic glandular hyperplasia does occur with unopposed oestrogens it can invariably be corrected with two 21 day courses of a progestogen. Scanning electron microscopy is being used to characterise the cell surface morphology in normal hyperplastic malignant endometrial cells."

Hormone replacement therapy.

Professor Frank Hytten, in discussion with Mr Geoffrey Chamberlain, talks about the metabolism of water during pregnancy. Two aspects of water metabolism are considered. The first centres on the fact that in early pregnancy plasma osmolality falls, a fact which is largely due to the effect of overbreathing - the mother avoids a state of diabetes insipidus by 'resetting' her osmo-receptors. The second is the phenomenon of water storage in pregnancy - an average of 2 1/2 litres in excess of that usually stored in known sites of tissue growth which, in some individuals, can rise to as much as 8 litres. They argue, however, that oedema is universal in healthy pregnant women and attempts to change it using drug therapy is dangerous to both mother and child.

Water metabolism in pregnancy.

A talk by Professor T. Chard from St. Bartholomew's Hospital Medical College. The following summary accompanies the videocassette: "Biochemical tests of fetoplacental well-being depend on the measurement in the mother of materials which differ from those in the nonpregnant adult. The two most popular tests at the present time are measurement of oestrogens and hPL. The main problems with these tests are the lack of information on the biochemical role of these materials and the very substantial overlap between normal and abnormal. Despite this, however, they have been shown to be of great value in a number of clinical situations and, in particular, in the diagnosis of intrauterine growth retardation. Recent developmetns include the introduction of new specific fetoplacental products, the evaluation of specific tests for placental transfer, and the use of biochemical tests as a screening procedure in all pregnancies."

Hormone and enzyme testing in pregnancy.

A discussion between Dr. Fiona Broughton Pipkin, University of Nottingham, and Mr. Geoffrey Chamberlain. The following summary accompanies the cassette: "Angiotensin II (AII) is an extremely powerful vascoconstrictor hormone, formed by the action of the enzyme renin on an -d-globulin substrate. Levels of renin and AII are high inn normal pregnancy but a specific compensatory diminution in vascular reactivity probably via PGEs, protects against hypertension. In hypertensive pregnancy (PIH), this protection is lost, and levels of AII are usually still further increased. There is a positive statistical relationship between diastolic BP and AII levels in pregnancy in humans and sheep. The administration of an AII receptor blocker to conscious pregnant sheep evokes a dose-dependent fall in blood pressure, the size of which is proportional to initial AII levels. There is also increasing indirect evidence that the feto-placental unit may contribute to circulating ALL levels. It is suggested that the relative ischaemia of the feto-placental unit in PIH results in increased AII production, as in the ischaemic kidney. This then results in increased PGE production in the uterus, which, together with the increased systemic BP improves feto-placental perfusion. This PIH may initially be a physiological response on the part of the feto-placental unit."
