    SUBSTANCE f OF A DECLARATION, Required to be made by or on the behalf of a perfon * who propofes to make an affurance on his or her own life. I A. B. born in the parifh of St. George, in the County of Middlefex, June the 6th, 1751 ,but now of IValthamJloWy in the county of Ejfex, being defir ous of becoming a member of the Society for Equi¬ table Affurances on Lives andSurvivorJhips, and in¬ tending to make affurance in the fum of upon and for the continuance of my own life, and having perufed and confidered that * claufe of the deed of fettlement of the faid fociety which re¬ quires a declaration in writing of the agey ft ate of health, and other circumftances attending the perfon whofe life fhall be propofed to be affured, do hereby declare and fet forth, 'That my age does not exceed thirty-four years \ and that I am not afflidled with any diforder which tends to the fhorteningoflife \ and I do hereby agree that this declaration be the bafts of the contrast between the Jaid fociety and me, and that if any untrue averment is contained in this declaration^ all monies which fhall have been paid to the fociety upon account of the affurance made in ccnfequence thereof, fhall be forfeited.
