Description

Register in form agreed to at annual meeting of the Association of Medical Officers for the Insane, 2-3 June 1842. Marked on Front: 'Association of Medical Officers of Hospitals for the Insane 1842' (see recommendations for filling in such registers at RET 1/5/5/2/1 Pre-printed columns and headings: patient number, name and residence, class in reference to rate of payment, sex, age, single or married, number of children, occupation, degree of education, profession of religion, habits of life, temperament, original disposition and intellect, duration of disorder on admission, apparent or alleged causes, form of mental disorder, particular propensities and hallucinations, accompanying bodily disorder, changes in the form of disorder before discharge, date of admission, date of discharge, result, cause of death, observations. With it are recommendations for filling it in and a sample page There is only one register in this series because after 1845 it was superseded by RET 6/2/3. A second register in this format is at RET 6/6/3/1, but this appears to have been used for a specific medical/statistical purpose rather than as a straightforward admission register, so it has not been included in this sequence
