Description
Lymph node: Epstein-Barr virus (EBV)-related lymphoproliferative disease
This is a high power view of the same lymph node biopsy seen in image W0047334. It shows a polymorphic infiltrate of lymphocytes, lymphoblasts and some multinucleate lymphoblasts. These are predominantly B cells. The polymorphous pattern indicates that this is not a lymphoma. H&E stain.
This pathology is related to EBV infection and is a frequent phenomenon in patients with advanced HIV disease. PCR analysis showed polyclonal heavy chain gene rearrangement confirming that this is not a lymphoma (monoclonality would indicate a lymphoma).
This lymphoproliferative process is analogous to that seen in patients with transplantation and EBV infection. It can be modulated by treating the HIV infection with antiretroviral therapy. Many patients go on to develop a genuine lymphoma from this condition.
