![GENERAL SCHEME. [Take each case according to this scheme, but if any par- ticular system is affected, amplify the notes according to the special scheme for that system.] The complaint for which the patient was admitted to the Hospital. I.—History of Present Illness. State the date when and how the illness began and the course it followed up to the patient’s admission, as much as possible in his own words. [If the patient is a child, or is unable to give a history, state from whom the history was taken.] Secondly, ask questions to elucidate points that are not clear in the patient’s statements. II.—Previous Health and Habits. Has he had any previous attack resembling the illness for which he is now admitted ? If so, give full details. Acute Specific Fevers. Scarlet Fever. Has he suffered from any accident or illness other than those mentioned above ? If the patient is married give the children in order. In case of a man ask if wife has had miscarriages.
