Certificate issued (in this instance) in December 1917 to No. 650564, C.L.M.I.H.I Maddocks of the 21st London Regiment. It is printed in red and black and shows 2 soldiers presenting arms to Britannia, sword in one hand, Union Jack in the other, a lion in the background. The royal coat of arms is above, pillars to either side.
