To do combined operations well within the time limits of safe antes- thesia, requires, in addition to the necessary degree of operative skill and dexterity, sufficient and efficient assistance, a perfected technique of the various operations attempted, and an instrumentarium suitable to rapid work. Perhaps, then, I may be pardoned a brief description of the technique of the various typical operations entering into combinations, as I have practised them, before giving attention to the combinations themselves.
