Leaflet about organ donation, common misconceptions about organ donation and a blank form for prospective donors' to register their wish to donate organs after death. The form can be detached and returned to the Department of Health. Also includes an organ donation card for people to carry if they choose not to register with the Centralised Organ Donation Register.
