The screaming patient, sufffering from ascites, sits on a sofa, left. A surgeon and a physician extract various organs from his body. Another physician carrying a jug walks towards them with an unsteady (drunken?) gait. Another surgeon picks his pocket. Right, a skeleton with a scythe, representing death
