What brought you to our website today?

A series of I, II and III programmes compiled together starting with an introduction to the case history of a patient, George Lister, who visits the doctor with his mother in 1940 aged 10 when they are interrupted by an air-raid. The examination proceeds in the shelter where he is prescribed bedrest for a swollen ankle. There is plenty of background information about the patient's lifestyle and back history. Thirty-six years later he returns to the doctor with pains in his chest. Part II resumes the story; the results of the chest x-ray and other tests are discussed by a doctor and consultant. He is prescribed beta blockers and a change in his lifestyle is recommended. Although he stops smoking he continues his stressful job as a ladies' panty-hose (tights) salesman. Six years later he has a tooth-ache; he is advised of the side-effects of anaesthetic and pain relief but later feels unwell and his wife calls the GP. In Part III, George feels better and goes out to celebrate. Unfortunately he over indulges in food and wine; subsequently he collapses and ends up in A&E. There is some debate about the exact nature of the problem with his heart. Whilst waiting for the consultant, he goes into cardiac arrest and dies.

Granuflex Hydroactive Granules is a new product by Squibb Surgicare which protects the surgical site. A number of American clinicians have researched the unique action of Granuflex. The benefits of the product are described. They have moisture reactive granules which protects the wound and prevents infection. The graphic images describe its action. A range of product booklets are available including a film 'An Environment for Healing' (not in the collection) also a film 'The Aetiology and Management of Leg Ulcers' (in the collection).

Liz Hudson, a young woman who is a keen competitive runner suffers from joint swelling and pain after exercise. After visits to her GP and the hospital over some time, her condition worsens, she eventually receives a diagnosis. The videos explore a number of alternative possibilities 1. Rheumatoid Arthritis, 2. Reiter's Syndrome, 3. Psoriatic Arthritis, 4. Ankylosing Spondylitis.

A film history of the struggle against infection after surgery. This is introduced by rostrum camera stills then, followed by a recreated sequence of Joseph Lister carrying out surgery including a time when the patient escaped from the operating theatre. The advent of anaesthesia revolutionised surgery, however, as the film goes onto articulate, there is still the scourge of sepsis or infection. Historical theories of infection are outlined including antiseptics. Asepsis is described and the story of how rubber gloves became adopted. There is a brief archive sequence of Alexander Fleming in colour smoking in his laboratory handling petri dishes. Sir Charles Illingworth and James Ross describe their first experience of antibiotics. The film describes the 'tragedy' of antibiotic usage; indiscriminate prophylactic use at low dosages. Surgeons, Mr Kennedy Browne and Mr David Rosin describe the decrease in efficacy of antibiotics. Professor Sidney Selwyn also contributes to this discussion. Staphylococcal infection in particular has risen through widespread use of broad spectrum antibiotics. Selwyn talks about drug resistant infections. Endotoxins are also outlined. There are surgical sequences throughout the film. Antiseptics and asepsis have returned in favour due to the 'casual' approach to infection in hospitals. In the closing sequences of the film, the opening historical sequences are revisited.

A partly-dramatised film produced for GPs about how to examine a newborn baby. A doctor is called out in the later stages of a normal home birth (the expectant father is left downstairs chain smoking with a tumbler of whisky). A consultant obstetrician at the Hammersmith Hospital shares his experience of examining new born babies; they should be examined twice, initially to rule out any life-threatening conditions and then later to identify any abnormalities. An initial examination is demonstrated on a new born male baby. A definitive examination is made about a week later with the mother present; the doctor carries out a home visit and another doctor follows the same procedure in hospital. A naked (and crying baby) is examined in detail (the presence of the mother is considered helpful). Examples of what to look are provided by footage and medical illustrations of various cases. Colour is important; problematic cynosis is illustrated. The identification of birthmarks is described. After the skin and body, the baby is exmained from head to foot from the mouth and palate,followed by the chest, the umblical cord, the anus, genitalia in boys and in girls and the feet. Vision and the eyes and the Moro reflex are studied to identify various abnormalities.

Part 6 of the series covered the following topics: Joint Replacement, Depression, Male Infertility, Medicine Today - V.D. Joint Replacement is presented by Mr. B. Wrobleweski, Consultant Orthopaedic Surgeon.; Depression by Dr M. A. Simpson, Senior Lecturer in Psychiatric Education at the Royal Free Hospital; Male Infertility by Mr. M. M. Usherwood; Medicine Today by Dr. J. K. Oates, Consultant Venereologist, Westminster Hospital. The programme takes the format of a studio based production with consultants interviewing patients with the relevant conditions. Joint replacement surgery is videoed for the first topic. The programme is interspersed with commercials for pharmaceutical products. 'Simon', a young patient with depression talks about his experiences which is videoed in the surgery; 'Peter' also talks about his experiences. A couple detail their problems conceiving; sperm under the microscope is seen. The consultant talks about the quality of sperm. Scrota are examined. An unidentifed woman has a cervical smear. The programme ends with a discussion about recurrent sexually transmitted disease.

Leva-dopa has revolutionised the treatment of Parkinson's; a patient is seen before with rigidity and after taking his medicine fully mobile and relaxed. Dr Andrew Lees at the neurology unit at University College Hospital, London, talks about Leva-dopa. There is a 'honeymoon' period and there are a number of long-term side-effects of the drug; these can be even worse then the original disease and are detailed. Wearing off symptoms are outlined. (Simon Nicholas appears briefly in shot about 9 minutes in). Dr Richard Hardie talks about the abruptness of some of the transitions especially in walking and why and how this occurs (shown via medical animations).

A one-off box set along the lines of The Clinico-Pathological Conference (CPC) series of programmes designed to provide the pharmaceutical industry who sponsored their production with educational material to promote discussion with the medical profession. Two of the three patients would benefit from treatment with Ranitidine (brand name Zantac); one won't. The video series gives the viewer the opportunity to decide which patient and video to watch.

A medical film which explains how pressure sores can form via restricted blood flow. There is a sequence of a ballet dancer dancing at the beginning; a motif to which the film returns. Photographs, infographics and intertitles are used. Immobile patients are described - they are unable to move naturally for a range of reasons and are particularly at risk. Pressure sores have four stages which are illustrated graphically. (The film was made to promote Granuflex dressings).

Surgicare has revolutionised stoma care; the film outlines some of the breakthroughs. Mr Nichols is a general surgeon at Hemel Hempstead Hospital and attends to a female patient who is experiencing pain; he explains her case history. She had an investigative barium enema which identified a potential fistula. Nichols recommends an operation followed by stoma. He narrates the operation. There are other forms of wound drainage packs from Squibb. Patients are happy with the products.

Surgicare in surgery (shortened version).

A training video about how care should be taken in diagnosing epilepsy in children with three cases of young people diagnosed when children. The viewer has a card and should indicate whether each child has epilepsy or another kind of seizure. The first patient, Rebecca, was four when first diagnosed and she was stable until ten years old when she presented some strange repetitive (and seemingly attention seeking) behaviour. A doctor finds no obvious physical cause. To be on the safe side they decide to use an ambulatory cassette recorder to monitor her EEG brain waves and an anomaly was identified. Sharon was noticeably 'vague', had seizures and was diagnosed with petit mal. Subsequently, several treatments were unsuccessful and she missed school. Mark was a normal child until eight, when he had night seizures. Further tests revealed some abnormal EEG readings. At the National Centre for Children with Epilepsy, Dr Gregory Stores, recaps on the cases and gives his opinion. There are several slides showing pathological specimens (courtesy of Dr J. T. A film which describes how the medium of film has been instrumental in the study of bacteria; a gram negative bacteria is shown magnified many times. Photographic techniques help in analysing the differing effects of antibiotics on bacteria. It is explained that there are two kinds of bacteria; bacteriostatic and bactericidal. The action of antibiotics is described. Timelapse photography shows e-coli multiplying and then after the action of different groups of antibiotics. Their cellular structure is illustrated graphically. Dr George Rollinson has led a research study looking at the speed of the action of antibiotics and the synthetic penicillins. Some of the research is carried out using video-ing and then subsequently using cinefilm. The specifics of their appearance are described over time after exposure to various agents; ampicillin, amoxycillin. The pros and cons of each antibiotic are summarised.

The film starts with a sequence of a light aircraft in flight doing manouevres; this introduces us to a patient who testifies about the ease in which he is using a stoma. A dancer also provides testimony; she had a colostomy and awoke needing a a colostomy bag. John Talbot, surgeon, Harold Wood Hospital, narrates an operation on the bowel preparing it for the stoma. The diseased bowel is discarded. Medical graphics illustrate the procedure. Talbot observes that it is the job of the post-operative staff to assist in the management of a healthy stoma. A female clinician briefs student nurses in the care of a stoma bag post-operatively. A slide show illustrates some problems which confront the stoma nurse; they are shown pre- and post-treatment. More detail is given regarding the siting of the stoma. Patients on the sound track describe their experiences. They describe the specialist stoma products which have liberated their lives. Tom W. Sturgeon, chairman of the Ileostomy Association, talks about the history of the organisation and how the association was established by clinicians to help their patients in their everyday lives as well as the day-to-day care of their stoma. Sue Ryder attends one of their AGMs. The AGM is shot by Dr Simon Nicholas; the chair asks people to smile when the camera turns their way. One of the pressing questions raised by the attendees about sexual satisfaction after the operation (answered by Mr York Mason). Ian Todd, St Bartholomew's Hospital, comments on the anxiety from patients pre-operatively. Mrs Joyce Parsons, a stoma nurse at Harold Wood Hospital, talks to a patient about his operation; she has had a stoma for 18 years. She reassures a younger male patient. Professor Bryan Brooke, founder and president of the Ileostomy Association comments about the continuing care provided by the health services. The dancers and pilot from the start comment on how well they now feel and how little the stoma affects their lives.

A woman is concerned about her husband who is presenting some aggressive, erratic behaviour; he is ranting at a neighbour in the garden. In A&E, the doctor sees him without his wife. He seems angry, confused and exhibits threatening behaviour. Dr Linda Gast, Reader in Psychiatry, pauses the story and indicates that the interview hadn't gone well. The purpose of the video is to stop violence; points where discussion would be useful are indicated by 'Stop Tape'. Dr Tom Brown, Consultant Psychiatrist, talks about safety issues, beginning with the environment (the room being accessible, private and not remote with an alarm/telephone etc.). Dr Siân Koppel, Specialist Registrar in Forensic Psychiatry, discusses how to prepare for the interview and gather objective information about the case. Dr Koppel goes through how an interview should be conducted. Dr Brown advises how to identify aggressive cues. Next, discussion is around how to assess risk, especially in the patient's case (he has delusional hallucinations about his neighbour). Some of the key questions to ask are outlined. Further investigations - family and GP should be consulted. When an admission is advised, how to effect this is outlined.

Part five of a series of five programmes about a patient, George Lister. This programme deals exclusively with the post mortem and the cause of death revisiting some of the key scenes in his case history from the previous parts. Pathological specimens are inspected. A patient who survived an operation on a ruptured septum is shown alive and well. Ends suddenly.

Historically the veins in the foot have not been considered to be important (starting with images of the foot drawn in detail by Leonardo da Vinci). However, Drs Michael Gardner and Roger Fox from Torbay Hospital have become interested in the venous return of the foot. In order to evaluate the venous pressure in the foot, they developed a vertical table to investigate this (shown photographically). The significant benefit to continuing with this research is how to stop thrombosis occuring when a patient is immobilised.

Parts one and two of this video outline David Marshall's case history; he is an overweight, stressed advertising executive who also smokes. There is a brief dramatised insight into his poor lifestyle at work and at home; he has just won an account for a well-known liquorice brand (which has relevance later). He visits his GP and sees a trainee doctor rather than his regular doctor. His blood pressure is high; he smokes 30 cigarettes a day and takes no exercise. His consultation seems perfunctory and the doctor is constantly interrupted by the telephone. He returns to the GP a few months later; his regular doctor asks some lifestyle questions and suggests that he returns about a month later to have his blood pressure checked, although the focus is on losing weight. It transpires that Marshall has been taking some but not all his prescription medicine. The doctor tries to get Marshall referred to the cardiac clinic, but can't get an appointment quickly. He has raised glucose levels and is still overweight. He has a mild heart attack in the street and goes to hospital. Part three is presented by T. W. Greenwood who goes over what we know about David Marshall. Greenwood makes some suggestions regarding Marshall's care. The video is preceeded by a Ciba Laboratories infomercial for Transiderm-Nitro. Part 1 is entitled 'Mr Simon Presents'. Mr Simpson is having tests on his heart in hospital on a treadmill with cardiogram. He recounts how he started to feel unwell. Part 2 is entitled 'Mr Simpson is referred to a rheumatologist' with the view that his shoulder plan in muscular rather than heart related. Mr Simpson describes his symptoms and then awaits the results of the tests. Still worried, he revisits the GP who carries out a resting ECG test in the surgery. Another series of tests are carried out - this time an exercise ECG. There are a number of cardiac and non-cardiac possibilities. The viewer is offered 1. Typical angina, 2. Atypical angina and 3. Cardiomyopathy. The symptoms are recapped. Real patients recount how they experienced similar symptoms. For one patient, an angiogram revealed a heart problem and he is offered an angioplasty. Another infomerical for Transiderm-Nitro is inserted. The symptoms are recapped for an alternative diagnosis; a patient outlines his symptoms. An angiography shows that the heart is normal but a barium swallow reveals a number of defects which are the cause of the patient's chest pain. Part 3; Typical Angina; the diagnosis is presented by Dr Richard Sutton, Consultant Cardiologist, Westminster Hospital.A patient outlines his symptoms. An angiogram as before is carried out. Finally a diagnosis of Hypertrophic Cardiomyopathy is offered. A patient with this condition describes the symptoms.
